- Complete New Fixed Assets Form pertaining to the same project.
- Complete Fixed Assets for Expansion and Development Form.
- Complete Fixed assets for Upgrade or Renewal Form.
- Complete Sale of Exempted Fixed assets Form.
- Complete Re-export of Exempted Fixed assets.
- Complete Transfer of Ownership or Merger.
- Complete Other Requests Application Form.
